Opt-Out Information for the Oregon Statewide Assessment System (OSAS)

In 2010, Oregon adopted higher K-12 standards in English Language Arts and Math to ensure all students move from grade to grade with the academic knowledge and skills necessary for success beyond high school.

To opt-out of state assessment, please complete the form below. There is a separate form for math/ELA and science. For science, parents can only opt out for a disability or religious beliefs. This form is only valid for the 2022-23 school year. Parents and adult students are required to submit an opt-out form annually should they wish to be exempted from statewide summative tests.

After completing the forms, they can be uploaded below (photos are acceptable) or brought into the Rock Creek or Sylvania ECHS office. 

Reading, Writing, and Math opt-out Forms:

English, Spanish, Chinese Traditional / Simplified, Vietnamese, Russian, French, Arabic, Tagalong, Japanese, Korean, German

Science opt-out forms Under Oregon Administrative Rule 581-022-1910, parents may request that their student be exempted from taking the Oregon State Assessment System (OSAS) Science Assessment to accommodate a students disability or religious beliefs.
